 Medical Policy, Emergency medical Forms and Immunization Records

Medication Policy:

If your child needs medication during school hours you must fill out a "parent's request to administer medication at school" form. If the medication is a prescription medication such as an inhaler, the doctor must also fill out and sign this form. 

Emergency Medical Forms:

Each year you must fill out an emergency medical form for your child. If during the school year your child develops a new allergy or illness or you have new telephone number, please contact the nurse so the form may be updated.

Immunization Records:

Your child may not attend school without a complete immunization record. Obtain a copy of the most current immunization record from your child's pediatrician and send it to the health room.
